Please read these terms of use (“terms of use”, “agreement”) carefully before accessing and using the CogDrisk assessment (“tool”, “website”, “service”) operated by the CogDrisk research study (“us”, “we”, or “our”).

Your access to and use of the Service is conditioned on your acceptance of and compliance with this Agreement. This Agreement applies to all users who log in or use the Service.

Intellectual property

You agree that all materials, products (assessment), and services on this website are the property of the CogDrisk research team (Professor Kaarin J. Anstey and her team) including all copyrights, trade secrets, trademarks, patents, and other intellectual property. You also agree that you will not reproduce or redistribute CogDrisk research team’s intellectual property in any way, including electronic, digital or new trademark registrations.

Statement about temporary data use

In order to calculate the risk score, we will need temporary access to the anonymized survey responses in your web browser. This data will, for a brief moment, be sent to our servers to calculate the response to send back to you. After that the data is erased and is in no way accessible by any staff member.

You understand that the CogDrisk tool is not intended to be used as a diagnosis tool for dementia. The information provided on this website and in the tool is for educational and research purposes only. If you have any concerns, you should consult your medical practitioner to receive specific medical advice.

Optional consent and user accounts

If you would like to check your dementia risk score and get recommendations for reducing your risk and do not want to participate in research, you can do so by opting for the ‘I wish to take the assessment and not participate in research’ pathway. If you are interested in participating in research, it is important that you read and understand the online participant information statement. You will need to provide your consent and some personal information such as your name and email address.

You have an option to create an account with us for the purpose of being able to log in and see your data any time you choose. When registering for this account, you will be asked to provide your name and email address. You are responsible for ensuring the accuracy of this information, and you are responsible for maintaining the safety and security of your identifying information. You are also responsible for all activities that occur under your account or password. If you think there are any possible issues regarding the security of your account on the website, inform us immediately so we may address it accordingly.

What type of personal information will be collected?

As previously mentioned, for individual accounts you will be asked to provide your email address. This is primarily used for signing into your account and correspondence from the application where required.
Analytics information will be collected from the website page for user engagement like:

  • the time and date of access
  • your internet protocol (IP) address
  • the pages accessed and any items downloaded
  • the type and version of the web browser you were using for access

Any analytics data captured by CogDrisk is stored securely, not used for identification purposes and is treated in accordance with section 8 of the NeuRA’s full Privacy Policy.

Information about you from the assessment will be stored in a de-identified format on a NeuRA secure server. Your assessment responses will be assigned a unique code that can be linked to your personal details. However, your personal details will be stored separately to your assessment responses and will be protected from linking by strict role-based access privileges. Only staff with the highest level of accesses will be able to link your assessment responses.
